Supported Employment

Community Engagement

Residential Services
We believe that everyone deserves a safe and nurturing home in a community of their choice. Our residential services are tailored to empower individuals living with disabilities to live as independently as possible.
Personalized Support: We assist with daily living tasks, from personal care to meal preparation, ensuring that each person can lead a fulfilling life.
Safe, Accessible Housing: Our homes are designed for accessibility, prioritizing comfort and independence for all our residents.
24/7 Care: Our dedicated staff provides the right level of support based on each person’s needs, whether that’s around-the-clock care, help at specific times of day, or remote support when it’s needed most.

Medicaid Alternative Pathways (MAPS) Program
Through the MAPS program, SJCS young people with intellectual and developmental disabilities who are transitioning out of school build skills, confidence, and independence as they move into adulthood.
The program features:
Enabling Technology: Tools and remote supports that promote independence in daily life.
Independence Coaching: Support for building safety, self-care, and money management skills using reminders, visuals, and tech.
Community Navigation: In-person support to explore local activities and build meaningful relationships.
Employment Innovation: Guidance on preparing for work, finding a job, and growing in a career.
Virtual Community Resource Mapping (VCRM): Helps individuals map out important places and resources to support community living.

Eligibility Requirements
To access services with St. John’s Community Services in Tennessee, individuals typically need to:
Be living with an intellectual or developmental disability
Be enrolled in a State or Managed Care waiver, such as:
Employment and Community First CHOICES (ECF)
CHOICES
MAPs
1915(c) Waiver
(Note: This waiver is closed to new enrollment but individuals already enrolled can transfer their services to SJCS.)
Have a referral through your Managed Care Organization (MCO)
