Remote Supports

We are tech-nically there for you utilizing technology that meets your needs to provide your with a safe and independent life.

SJCS meets the person where they are and completes a technology assessment to identify the person and their team goals and outcome. Along with the technology assessment SJCS completes a Matching person with technology assessment (MPT) as well to ensure the person gets technology that works for them. 

SJCS also provides continuous follow-up to address any needs that may come up after the technology has been introduced.

SJCS also provides continuous training to the person and support team to ensure everyone knows how the technology should be used.

SJCS provides troubleshooting support remotely and or in person if the person or team runs into issues with technology.

MAPS

MAPS services is an innovative approach that focuses on a person-centered process.  Remote supports and technology are utilized to assist people in being more independent in their home, work, and community. 

Travel Training is provided in the MAPS program that assists the person supported in utilizing public transportation by creating routes within the Wayfinder app.

Certification in Advanced Virtual Community Resource Map where people are assisted in creating a map of places in their community that are important to them.  VCRM mapping creates an opportunity to work with people to add resources to assist them in being more independent in their community.

  • Community Navigator - travel training in the community to obtain relationships and find activities they enjoy

  • Enabling Technology - an innovative approach to providing services using technology as a bridge in gaping services by providing remote supports in the home, work, and community

  • Independence Coaching - an innovative approach to obtaining safety skills, health and financial management, and self-care skills through reminders, visual, and technology

  • Employment Innovation - an innovative approach to pre-employment, employment, and career development 

  • Virtual Community Resource Map - mapping out your life with resources in your community to assist you in being independent

The Medicaid Alternative Pathways to Independence program in TN also known as MAPS exists to assist people in being more independent in their home, work, and community.

Training & Presentations

The training and presentation work SJCS performs is broken into two key groups.

For the people we support: We provide ongoing training on the technology used as a part of an onboarding process as well as ongoing as needed for the individuals and their support teams. 

For our partners and as contractors: We educate government agencies and committees to further person-centered tech support options. SJCS works as an industry leader in the assistive technology space and present at relevant conferences as subject matter experts.

(Innovative Technology):

“Enabling Technology (ET) is the use of various forms of devices and tech to support a person with disabilities to live as independently as possible.  These types of technologies include sensors, mobile applications, remote support systems, and other smart devices. Enabling Technology can support a person in navigating their jobs and communities, gain more control of their environment, and provide remote support and reminders to assist a person in independent living.”

“Remote support (RS) means the provision of supports by staff of an agency provider at a remote location who are engaged with an individual through equipment with the capability for live two-way communication.
