Residential Services
We believe that everyone deserves a safe and nurturing home in a community of their choice. Our residential services are tailored to empower individuals living with disabilities to live as independently as possible.
Personalized Support: We assist with daily living tasks, from personal care to meal preparation, ensuring that each person can lead a fulfilling life.
Safe, Accessible Housing: Our homes are designed for accessibility, prioritizing comfort and independence for all our residents.
24/7 Care: Our dedicated staff provides the right level of support based on each person’s needs, whether that’s around-the-clock care, help at specific times of day, or remote support when it’s needed most.

Day Support Services
Our Day Support Services provides person-centered support designed to promote skill-building, independence, and inclusion. Services are tailored to meet each individual’s preferences, needs, and goals.
Community Engagement: Structured, group-based support in community settings that allow individuals to build relationships, access resources, and develop independent living skills. Activities may include volunteering, exploring personal interests, and participating in recreational, educational, or cultural activities.
Center-Based Day Program: A safe and supportive environment for individuals who require medical oversight, mobility accommodations, or prefer structured onsite activities. Services include personalized skill development, socialization, and recreational engagement. Activities are designed to promote choice, independence, and daily fulfillment, while also supporting health and safety needs.
Community Coaching: One-on-one, time-limited support for individuals who benefit from intensive assistance to acquire, retain, or improve skills related to community integration. Services are individualized and focused on achieving specific outcomes, such as using public transportation, accessing community resources, or developing social interaction skills, with the goal of increasing independence and transitioning to less intensive support.

In-Home Supports
Our In-Home Supports are expertly designed to assist adults living with disabilities in their daily routines, fostering independence and significantly enhancing quality of life. These services are customized to align with the unique needs and preferences of each individual.
Personal Care Assistance: Compassionate support for essential activities like personal hygiene, dressing, etc.
Household Management Help: Assistance with meal preparation, cleaning, and organization for a more manageable home environment.
Health Monitoring Support: Help with medication reminders and health-related tasks to promote well-being.
Companionship: Engaging in activities that nurture mental and emotional well-being.

Therapeutic Consultation
Our behavioral consultation services are designed to help community members understand and manage behavioral challenges in a supportive, non-judgmental environment. We collaborate with individuals and their families to create effective strategies for addressing behavioral concerns, promoting positive behaviors, and enhancing overall quality of life.
Behavioral Assessments: In-depth analysis of behavioral patterns to develop a clear understanding of challenges and needs.
Individualized Plans: Custom behavior plans that focus on skill-building, managing emotions, and promoting positive behaviors.
Support and Training: Providing caregivers with tools and strategies to support their loved ones effectively.
Crisis Management: Immediate support during times of behavioral crisis to ensure safety and stability.

Transportation Services
SJCS partners with Modivcare to provide safe, reliable, and accessible transportation for individuals receiving Medicaid waiver services across Virginia. Transportation is a critical support that ensures individuals can access community-based and center-based services, health appointments, employment, and other essential destinations.
Medicaid-Funded Access
Transportation is available to eligible individuals enrolled in the DD Waiver or CCC Plus Waiver and is arranged in accordance with DBHDS and Medicaid requirements through Modivcare, the state’s designated Non-Emergency Medical Transportation (NEMT) broker.
Community and Day Program Access
We coordinate rides to and from SJCS day support services, medical appointments, and other ISP-identified community activities to promote full participation and inclusion.
Safe and Person-Centered Travel
Modivcare transportation providers are trained to support individuals with disabilities and follow all applicable safety, accessibility, and documentation standards. Vehicles are equipped to accommodate a variety of mobility needs.
Scheduling and Coordination Support
SJCS staff assist with coordinating and confirming transportation through Modivcare, helping ensure timely pickups, accurate documentation, and continuity of services.

Eligibility Requirements
SJCS provides residential and community-based support through Virginia’s Home and Community-Based Services (HCBS) waivers. These services are designed for people with intellectual or developmental disabilities who need extra support to live as independently and fully as possible.
You may be eligible if you:
Have a documented intellectual or developmental disability
Need help with daily living tasks (such as personal care, cooking, or managing medications)
Complete a screening through your local Community Services Board (CSB) and are approved for a waiver
Are assigned a waiver slot through the state’s waiting list process
We primarily support individuals through the following waivers:
Community Living Waiver: For people who need 24/7 residential support, such as in a group home
Building Independence Waiver: For adults living on their own who need less frequent support
Family & Individual Supports (FIS) Waiver: For people living with family or independently who need in-home or community-based support
