Nursing Consultation Services
We provide personalized nursing consultation to community members. Whether it's coordinating care or providing resources, our nursing team is here to ensure that individuals receive the highest level of care, tailored to their specific needs.

Eligibility Requirements
To receive services in Delaware through SJCS, individuals must meet specific eligibility criteria set by the Division of Developmental Disabilities Services (DDDS). Here’s a simplified overview:
General Requirements
You may qualify if you:
Have a diagnosis of an intellectual or developmental disability (including autism, brain injury, or Prader-Willi Syndrome) that began during the developmental period
Are age 12 or older
Have limitations in daily functioning
Require a level of care typically provided in an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID)
Can be supported safely in the community with waiver services
Meet income and resource guidelines (generally under 250% of the SSI benefit rate)
Depending on the type of support you’re looking for, you may need additional requirements.
How to Apply
To get started, individuals or their legal guardians should contact DDDS Applicant Services. Once all necessary documents are submitted, DDDS typically makes a decision within 30 days.
